Jeffrey Scott, MDPalo Brea Pain & Rehabilitation

Dr. Jeffrey Scott, MD, received his Bachelor’s degree in Biomedical Engineering from Northwestern University in Evanston, IL. He then completed his graduate studies at Southern Illinois University’s School of Medicine in Springfield, IL, and continued on to complete his residency at Eastern Virginia Medical School in Norfolk, VA.

After becoming Chief Resident, Dr. Scott was board certified in Physical Medicine and Rehabilitation in 2001. In 2010, he became a Certified in Independent Medical Evaluator. As a Physiatrist in private practice, Dr. Scott believes that the care that goes into quality pain management is much more than pain medication and injections. It is the willingness to explore various treatments and evaluating the effectiveness of those treatments in the overall success and functional improvement of the patient. Quality care also involves assessing how pain can cause damage in areas such as self-independence, self-reliability, work issues, depression and the social stigma of disability and handicap.

Dr. Scott understands the physical and psychological barriers that chronic pain conditions create for the patient and their families. His definition of improvement is functional improvement. Maximizing function includes not only pain control but also patient education, communication and participation. With an individualized treatment plan, the impact of chronic pain on a patient’s quality of life can be minimized.


Joshua Levy, DOadmin-ajax11


Dr. Joshua Levy is originally from Plainview, NY. He attended college at SUNY Buffalo and received his Bachelor’s degree in Psychology while graduating Magna Cum Laude with highest honors. In 2001, he earned his medical degree from New York College of Osteopathic Medicine.

He completed his Physical Medicine and Rehabilitation training at Nassau University Medical Center in Long Island, NY. Dr. Levy then continued on to complete a Sports Medicine and Interventional Spine fellowship at UMDNJ in New Jersey. During this time, he was Assistant Team Physician for both Rutgers University and New Jersey Institute of Technology. Dr. Levy is board certified by the American Board of Physical Medicine and Rehabilitation and by the American Osteopathic Board of Physical Medicine and Rehabilitation.

Dr. Levy brings great enthusiasm to the practice of medicine. His holistic approach to patients with physical impairments, disabilities and pain gives him a unique perspective that strengthens his commitment to provide his patients with compassion, understanding, and education.

Eileen has been in the healthcare field for the last 35 years. She graduated from Northwestern University’s Medill School of Journalism in 1978 with a degree in Communications. She went on to further her education in the medical field and graduated from William Rainey Harper College in 1982 with an Associates in Nursing Science. She has worked in Pediatric and Adult Neurology and OB/Gyn. She received her Certification in Medical Management in May, 2008. She has been with the office for six years and enjoys the day to day management of a busy Physiatry practice. She enjoys traveling, reading, shopping and spending time with her grandchildren.

Perla has been with our office for the past two years. She graduated from Everest College in 2009 with her Medical Assisting Certification. She is currently enrolled in Arizona State University’s nursing program. She is in charge of the front and back office and ensures that the office runs efficiently. She is a married mom of two energetic sons and enjoys camping, hiking and traveling.

Mayte is our newest team member, joining our office in July 2015. She runs the front desk and coordinates referrals, authorizations and scheduling. She graduated from Everest College with her certification in Billing and Coding in 2011. She attended Maricopa Corporate College and received her Pharmacy Tech Certification in 2015. She enjoys working out, listening to music and shopping.



Palo Brea Pain & RehabilitationGraciela Calixto, Medical Assistant

Graciela is our back office medical assistant, coordinating care and follow up for our patients. She collects and documents clinical information and handles authorizations. Graciela graduated in 2015 from Carrington College with her Medical Assisting Certification. She has prior experience in the urgent care setting with a focus in industrial injuries. She joined our office in June 2015. She enjoys traveling and spending time with her family.



Palo Brea Pain & RehabilitationElisabeth Torres, Pharmacy Technician

Elisabeth graduated in July 2013 with a Pharmacy Technician certification. She previously worked for two years at a CVS Pharmacy in which she gained extensive knowledge in the field. She joined our staff in May 2015 and handles all of the prescription needs of our patients. She is a busy mom of an active 6 year old daughter. She enjoys reading, dancing and spending time with family.
